Maxillofacial Surgery


Effective July 31, 2019, I will no longer be practicing Oral and Maxillofacial
Surgery at 8 Baywood Drive, Queensbury, NY 12804. I plan to continue professional
activity in this area ; these plans and affiliations have not been finalized and details
will follow. In addition, I will continue my position on the NYS Board of Dentistry.
Please receive future care from a qualified oral and maxillofacial surgeon.
I believe that in the Capital District, the offices of Dr. Seth Farren (OMS) and
Dr. Stephen Dweck (Implantology) would provide you with competent, caring,
ethical, and supportive care. Consult with your current dentist for other suggestions.
My office records will be available for a period of time ( NYS BOD regulations);
please contact my office by July 19, 2019, to obtain copies of these medical records.
As required by NYS Regulations, a records release authorization is necessary.
If you choose to pick up the copy of these records, please make your request by
July 19, 2019. After this date, these records will be will be held in a secure location
and retained before being properly destroyed according to NY State regulations.
